Coop Ed. Required Forms

  1. Cooperative Education Learning Agreement - this form is to gain agreement among the student intern, the site supervisor, and the faculty coordinator on mutual intentions, expectations, and the techniques used for evaluation. This form is to be completed  BEFORE the cooperative experience begins.

  2. Cooperative Education Statement of Learning - his form is to be completed by the student intern and the faculty coordinator. It is composed of the learning objectives for the student's specific cooperative education experience. This form is to be completed  BEFORE the cooperative experience begins.

  3. Academic Honesty Policy Statement - this form is to be completed by the student intern BEFORE the cooperative experience begins.

  4. Cooperative Education Work Log - this form is to be used by the student intern to keep track of his/her site hours. This form must be signed by the Employer/Supervisor and the student intern at the end of the learning experience. Download the form, keep a daily record, and turn the Work Log in to Melody Head at the Career Service office (SHH301) when the cooperative experience has been completed.

  5. Site Visitation Form - this form  will be completed by the faculty coordinator during the semester the student intern is participating in a cooperative education course. This form is to be completed at the conclusion of the cooperative experience.

  6. Employer's Evaluation - this form must be completed by the employer/supervisor at the completion of the student's required hours and learning experience. This form is to be completed at the conclusion of the cooperative experience.

  7. Student's Evaluation - this form must be completed by the student at the completion of the required hours and learning experience. This evaluation form reflects the student's assessment of their cooperative education experience.
