Cooperative Education Paperwork  

All paperwork must be submitted to the Office of Career Services.

Application & Resumé:
The application must be completed and returned to Career Services with a copy of your most recent resumé. Satisfactory completion of this form does not obligate the student in advance to a specific placement, nor does it obligate the college to place the student. It does signal the beginning of a process to try to arrange a mutually satisfactory cooperative education experience.

  • This form must be signed by the student, the faculty coordinator, and Career Services.

CE 5 - Petition for Credit:
The Petition for Credit outlines relevant information about the student, the cooperative education employer, and the cooperative education position.

  • This form must be signed by the student, the faculty coordinator, and the employer/supervisor.

CE 7 - Statement of Learning Objectives/Syllabus:
This form must be completed by the student and the faculty coordinator. It is composed of the learning objectives for your specific cooperative education experience and how you plan to fulfill these objectives. The Maximizing Learning section of the handbook will be beneficial to you as you complete this form. This completed form will become your cooperative education syllabus.

  • This form must be signed by the student and the Faculty coordinator.

CE 11 - Site Visitation Form:
All faculty coordinators will complete at least one on-site visit with the student each semester. The faculty coordinator, the employer/supervisor and the student will discuss how the cooperative education objectives are being met.

  • This form must be signed by the student and the employer/supervisor.

CE 9 - Employer’s Evaluation:
This form must be completed by the employer/supervisor at the completion of the student’s required hours and learning experience. It is the student’s responsibility to see that the Employer Evaluation Form (CE 9) is given to the employer and returned to Career Services by the stated deadline. The evaluation form provides feedback from employer/supervisors on how the student performed in their cooperative education experience.

  • This form must be signed by the employer/supervisor and the student.

CE 13 - Student’s Evaluation:
This form must be completed by the student at the completion of their required hours and learning experience. This evaluation form reflects the student’s assessment of their cooperative education experience.

CE 15 - Cooperative Education Log:
This form is to be used to keep track of your onsite hours. You must put in a minimum 30 hours for each hour of credit (90 hours for 3 credit hours).

  • This form must be signed by the employer/supervisor and the student.
